POSITIVE VIBES HEALTHY LIVES
Get involved in redesigning health and wellbeing activities for 12 to 16 year olds in Whitestone.
Our staff at Whitestone Surgery are proud of the NHS and the overall services it has to offer. However we do realise any service can improve and that’s why we have chosen to explore, with the help of parents or guardians and their children, services that will facilitate the wellbeing of the younger person. We want to encourage young people to be involved and work with us to identify and develop the activities for their benefit.
We are very fortunate to have working with us, Hannah King, an MSc Psychology graduate, who is interested in working with this age group. Hannah has offered to work in association with our award-winning Patient Participation Group, to gather feedback from our younger patient population.
We hope that you will be happy to give your permission for your child to help us explore and redesign our health and wellbeing activities to better suit the needs of young people between the ages of 12 to 16 years old.
If you support this initiative and are happy to be involved, could you kindly ask your child to fill out the survey online here. We also ask if you could give the link to the survey

to another parent/guardian of a child within the Whitestone community. Good news!! The family does not need to be registered with this surgery to participate.
You have the option to complete the survey anonymously. However, if you do identify yourself, it will give Hannah an opportunity to gain further insight from you, or your child on how we can improve. I will ensure all who have participated in the survey are specifically contacted to give overall feedback on what we did with the survey information. To view our privacy policy please click here.
In advance thank you for helping us to deliver this worthwhile programme. If you have any questions, please contact our reception.
