A guide for Whitestone residents over 50 aimed at reducing loneliness and isolation and creating better health and wellbeing.

Project Summary

Happy, Healthy and Involved (HH&I) is a community-led initiative founded by the award-winning Whitestone Surgery Patient Participation Group (WS-PPG), working in partnership with Whitestone Surgery, Warwickshire County Council, Guideposts, Volunteer Friends, and the University of Oxford CEBM Outreach team.

The project was created to reduce isolation and improve wellbeing, particularly among residents aged over 50 who were at risk of marginalisation. HH&I brought together local government, NHS services, voluntary organisations and community groups to better connect residents with support, activities and opportunities within their local area.

What HH&I Did

  • Led by an award-winning patient participation group
    The Whitestone Surgery Patient Participation Group, recognised nationally for its community work, played a central role in founding and shaping HH&I. The project was guided by research carried out by Whitestone Surgery into quality of care across different population groups, including a local survey of residents aged over 50.

    The findings highlighted gaps in access, information and social connection. In response, HH&I helped develop a Directory of Community Services, which was distributed locally to over-50s, signposting activities such as volunteering and social groups, alongside practical support including transport, mobility services, GP access and other local sources of help.

  • Built strong cross-sector partnerships
    Worked collaboratively with Warwickshire County Council, NHS services, Guideposts, Volunteer Friends, the University of Oxford CEBM Outreach team and other local organisations.

  • Connected residents to local services
    Developed and distributed a Directory of Community Services to help residents easily access health, wellbeing and community support.

  • Promoted volunteering and social involvement
    Partnered with Volunteer Friends, U3A and community groups to help residents find meaningful ways to get involved.

  • Supported health, wellbeing and active lifestyles
    Linked residents to walking for health, wellbeing initiatives and public health programmes across Warwickshire.

  • Reduced isolation for older residents and carers
    Supported social programmes such as the Caring Café and community events that created welcoming spaces for connection.

  • Improved digital inclusion and skills
    Supported community computer groups and one-to-one learning to help residents gain confidence with basic digital skills.

  • Encouraged employment and skills development
    Supported local job clubs and initiatives aimed at unemployed and under-employed residents.

  • Raised funds and celebrated community action
    Helped support fundraising activities for charities including Save the Children and The Silver Line.
